Please note, that you must bring the form, signed and completed by your parent(s) or legal guardian to the event, or the bus with your chaperone, or you will not be able to participate. NO CHILDREN UNDER 8 ARE PERMITTED TO PARTICIPATE. NO ANIMALS ARE PERMITTED ON THE YOUTH DAY ATHLETIC FIELD.
